Program Details
Project Baby CheckSiskiyou Community Health Center, Josephine County Home visits, medical screening, and weekly support groups are offered to high-risk parents with multiple risk factors in isolated rural areas of Illinois Valley and Josephine County. Program provides medical case management, which includes well-child exams and immunizations in conjunction with the social/ emotional/educational services of the home visitor utilizing the Parents As Teachers curriculum.
